Gorou Amamiya, an obstetrician in Takachiho, adores Ai Hoshino, the sixteen-year-old star of the idol group B-Komachi, a devotion he inherited from Sarina Tendouji, a dying girl once in his care. When Ai arrives secretly pregnant with twins, he agrees to deliver them, but on the night of the birth her stalker pushes him off a cliff. Gorou wakes as Ai's son, Aqua, his memories intact. His twin sister, Ruby, is Sarina reborn. Neither knows who the other was.
Ai hides her children and becomes a star. On the day she is due to play the Tokyo Dome, the stalker stabs her at her home; she dies telling the twins she loves them, and he kills himself. Aqua works out that someone close to Ai gave the stalker her address, and suspects their unknown father.
Ten years later Ruby wants to be an idol, and Aqua takes acting work to reach people who knew Ai. A drama reunites him with Kana Arima, a faded child star. On a dating reality show he meets the stage actress Akane Kurokawa; edited into a villain and buried in online abuse, she is about to take her own life when Aqua reaches her. Afterwards they fake a romance for the cameras. Ruby revives B-Komachi with Kana and the streamer MEM-cho.
In the second season, as Kana and Akane compete on stage, Aqua DNA-tests the cast. The lead, Taiki Himekawa, is his half-brother, and the man Taiki believes to be his father died long ago. Thinking his revenge is over, Aqua lets himself love Akane, while Ruby, finding Gorou's skeleton in a cave near Takachiho and learning that his killer had an accomplice, turns to revenge herself.
In the third season Akane identifies the real father: Hikaru Kamiki, once an actor in her own troupe, sexually abused as a boy by Taiki's mother, the actress Airi Himekawa. When a tabloid photographs Kana at the office of a director who made a pass at her, Aqua buries the story by handing it the secret of Ai's children. He breaks with Ruby, leaves Akane to keep her safe, and with the director Gotanda sets up a film of Ai's life, The 15-Year Lie, to expose Kamiki. Kamiki kills the actress first cast as Ai. Ruby wins the role, and Kana leaves the group to act. When Ruby falls apart at the sight of the mother who abandoned Sarina, Aqua realises who she was and tells her he was Gorou; the twins make peace. The season ends a week before filming, with Ichigo warning that Aqua is deteriorating. The anime has reached the middle of the manga's thirteenth of sixteen volumes; a fourth and final season is announced.

Gorou, an obstetrician devoted to the idol Ai, is killed by her stalker on the night of her twins' birth, and wakes as her son. After Ai is stabbed to death in front of him, Aqua decides someone close to her gave the killer her address, and hunts for their father. He sabotages Ruby's auditions, takes acting work to reach people who knew Ai and uses Akane's gift for reading people. When a DNA test seems to end the search, he lets himself love her; then he learns his father is alive. He trades the twins' secret to bury Kana's scandal, leaves Akane to keep her safe and sets up a film to expose Hikaru Kamiki. When Ruby breaks down, he recognises Sarina and tells her he was Gorou. A week before filming, Ichigo warns that he is deteriorating.

Ruby Hoshino’s story
Sarina Tendouji spends her short life in a hospital bed, adoring the idol Ai and in love with her doctor, Gorou, and dies at twelve. Reborn as Ai's daughter, she holds on to Sarina's dream after her mother's murder and, once a scout finds her, revives B-Komachi with Kana and MEM-cho. In Takachiho she finds Gorou's skeleton in a cave, learns that the man who killed him and Ai had an accomplice, and turns to revenge. Coached by Ichigo, she builds a persona and stages a controversy to make herself famous. When Aqua hands their secret to the press without asking her, she disowns him. Cast as Ai in The 15-Year Lie, she sees the mother who abandoned Sarina raising a new family and breaks down. Aqua tells her he was Gorou, and the twins make peace.

Ai Hoshino’s story
Ai grows up in a children's home after her mother, arrested for theft, never comes back for her, and tells the manager who scouts her that she does not know how to love anyone. He answers that saying it, even as a lie, might one day make it true. At sixteen she is pregnant with twins. She gives birth in secret, passes the children off as her manager's and goes back to work, keeping up lies she calls the best kind of love. She becomes a star and asks the director Gotanda to film her as she really is. On the day she is due to play the Tokyo Dome, her stalker stabs her at home. She speaks kindly even to him, tells Aqua and Ruby she loves them, and knows as she dies that this is no lie.

Why it matters
Oshi no Ko begins as a fable about a fan reborn as his idol's son and becomes a long look at what Japan's entertainment industry asks of the young people it sells. An idol must seem to belong to no one, so a pregnancy is hidden and a boyfriend is a scandal; devotion turns into stalking; a reality show edits a teenager into a villain and lets strangers do the rest. That arc was widely linked to the 2020 death of Hana Kimura, a wrestler who appeared on the reality series Terrace House. The author said it had been planned before she died, and when the anime aired it in May 2023 her mother, Kyoko Kimura, publicly criticised its makers. Beneath the revenge plot is a quieter question: whether a lie told out of love is still a lie, and whether a child can have a life of her own inside her mother's legend.
